The DSS Special Follow-up Codes are organized in alphanumeric order by Code.

| D1 | Eligible for one month diversionary assistance. |
| D2 | Eligible for two months diversionary assistance. |
| D3 | Eligible for three months diversionary assistance. |
| D4 | Eligible for four months diversionary assistance. |
| W1 | Entitled to first 6 mo of 12 mo extension & needs the 1st notice earnings rpt |
| W2 | Entitled to continued extended Medicaid cvg & needs the 2nd notice earnings rpt |
| W3 | Entitled to cont. extended Medicaid cvg & needs the 3rd notice eamings rpt |
| W4 | The case is entitled to the last (4th) phase of the extended Medicaid cvg |
| WA | Comp gen'd replace W1. First notice earnings rpt sent -5th day of 3rd ext mo |
