Futures Planning: Helping Communities
to Welcome People with Disabilities

Overview
By providing training in person-centered approaches to support, this initiative promotes practices that give individuals and families greater choice and control in their lives. The initiative encourages communities to support individuals by building a community network of leaders and supporters. The initiative is grounded in the principles of self-determination and strives to increase opportunities for freedom and autonomy. (Funded by
Administration on Developmental Disabilities, U.S. Department of Health and Human Services.)

Major Activities

  • Provide facilitation in person-centered approaches to individuals, their families, and friends
  • Introduce person-centered approaches to individuals, families, and professionals in several communities in Virginia
  • Offer training in person-centered planning tools to teams that include individuals, family members, agency providers, paraprofessionals, direct service providers, and administrators
  • Support family members from local communities as project staff
  • Link with other initiatives that support self-determination and consumer-directed supports
