A resource for transitioning students with disabilities
My Life Beyond High School

Plan

Now that you have a clear picture of the future you are aiming for, you can create a plan or "road map" to get there. This web site will help you develop your plan by providing you with information about resources available to you while you are in school and after you finish school. As you make decisions, you can save the information and print it out.

Your plan will need to be flexible enough to change as you grow and change, but specific enough to provide you with guidance about actions you should be taking now to move you toward your goals: what classes to take in school, what skills to develop, how to get on waiting lists for housing and other resources, and how to apply for funding or save money for your future. Ideally, you should begin planning for your future at the age of 14. Whatever your age, NOW is a good time to begin.

Your plan will form the basis for your Individual Education Plan. A form based on the IEP is available for you to record your choices.

Your plan will need to address these three major areas:

  1. Career
  2. Home and Community Life
  3. Benefits and Financial Assistance
