information is requested when scheduling an appointment?

scheduling an appointment, the following information is helpful:

Are you a new or existing patient?
How long have you had this problem?
Are you being referred by another doctor, or have you seen
another doctor regarding your condition?
If so, what was the diagnosis?
Do you have xrays or any other diagnostic testing results?
Was this related to a car accident or work-related incident?
Insurance information (e.g. referral, pre-authorization,
etc.)
Do
you only treat athletes at the Sports Medicine clinics?
Our clinic is Sports Medicine oriented, but we do not only treat
athletes. We treat individuals with orthopedic injuries who have
injured themselves at work, at home and at play. Our services are
not just for athletes.
Furthermore, we believe that there is a little bit of athlete in
everyone.
What
should be brought to the first appointment?
X-rays, MRIs or results from any diagnostic testing that you
may have
Insurance card and information
Records from any other treating doctors if available
Completed paperwork (forms) that can be printed from our
website forms page
Insurance referral and valid insurance card
Comfortable attire
What
if I forget or am unable to get an authorization in time for the Appointment?
(for patients with Managed Care Insurance only)
Not having an authorization at the time of the appointment impacts
our ability to provide quality care, especially if you need further
diagnostic testing and/or surgery.
It is ultimately your responsibility for making sure that an authorization
is obtained before your visit and for subsequent visits.
If you come to our office without the appropriate authorization
you will have the option of rescheduling your appointment and or
paying for the visit up front.
