Infant & Toddler Connection of Richmond Forms

Pre-Registration Form (PRE 07-03 A)

This form should be completed for all infants and toddlers (birth to three) who are deemed likely to be found eligible for early intervention services under Part C of the Individuals with Disabilities Education Act (IDEA).  This form is necessary to assure accuracy in the number of infants and toddlers receiving Part C services in the city of Richmond; to identify other public and private providers who may also be involved with the family; and for determining the family’s access to Part C funding.

Application for Evaluation & Assessment and Direct Service Funds (EADS 07-03A)

Service Coordinators may use this form to request funds for Part C evaluations and assessments and Part C direct services in accordance with Infant & Toddler Connection of Richmond policies and procedures.

Application for Family Support Funds (FS 07-03A)

Service Coordinators may use this form to request funds for family support in accordance with Infant & Toddler Connection of Richmond policies and procedures.

Application for Conference & Workshop Funds (CWF 07-03 A)

Individuals involved in supporting the work of the Infant & Toddler Connection of Richmond and families who have received system services are eligible to apply for funds to support their participation in various conferences and workshops.
