Acute Inpatient Care

The VCU Children’s Medical Center receives over 4,000 acute care admissions per year. Our 60-bed pediatric floor includes units for children from infancy to 21 years of age.  Our pediatric inpatient team serves children with every imaginable diagnosis, such as asthma, cancer, cardiac problems, cystic fibrosis, diabetes, infectious diseases, neurological and gastrointestinal disorders, as well as injuries sustained from traumatic events.

Adolescent Medicine

The Adolescent Health Service provides primary care for adolescents and young adults ages 12 to 21 years with an emphasis on comprehensive health education and anticipatory guidance for patients and their parents. Services include physical exams, diagnosis and treatment of acute medical problems, diagnosis and treatment of adolescent gynecologic problems and diagnosis, treatment and prevention of the consequences of adolescent sexual behavior, and assessment of common emotional/behavioral problems. A consultant in Pediatric and Adolescent Gynecology is available and participates in some clinic sessions.

Allergy and Immunology

When children suffer from acute or chronic conditions such as asthma, hay fever, eczema, food allergies or drug reactions, the skills of a pediatric allergy/immunology specialist are available at the Children’s Medical Center. Headed by Dr. Ann-Marie Irani, a leader in clinical research with more than 20 years of experience, the Division of Allergy and Immunology offers a fully comprehensive approach to the child with allergies, immune deficiencies and rheumatologic disorders. Specialists focus on clinical research areas in pediatric asthma, as well as basic research of the biology of mast cells and basophils.

The Division of Pediatric Allergy/Immunology offers diagnostic expertise in the following areas:

Pediatric Cardiology

The Division of Pediatric Cardiology provides full pediatric cardiology services including preventative and adult congenital heart disease services. The division’s physicians provide transtelephonic echocardiographic services in Richmond, Newport News and Fredericksburg. The Division of Pediatric Cardiology is the only National Triple Accredited pediatric echocardiography laboratory (fetal, trans-thoracic and trans-esophageal) in Virginia. The pediatric cardiac catheterization laboratory continues to provide state of the art diagnostic and interventional services for infants, children and adults with congenital heart defects, using such tools as implantable devices and lasers. Electrophysiology services such as RF ablation and pacemaker therapy are provided in association with the Electrophysiology Division of VCUMC. Cardiothoracic surgery is performed at VCUMC by Dr. Ben Peeler on infants and children with CHD and by Dr. Vig Kasirajan on adults with CHD.

VCUMC cardiology specialists provide outpatient services, including echo and ECG at the Children’s Pavilion as well as at St. Mary’s Hospital, Hanover Medical Park, Stony Point Ambulatory Care Center, Chippenham Hospital Medical Park, St. Francis Medical Park (all in the Richmond area), the Children’s Hospital Pediatric Therapy Center near Mary Washington Hospital in Fredericksburg, VA, and the Riverside Medical Office in Newport News, VA. Fetal cardiology consultations and echocardiographic consultations are performed at VCUMC as well as at the request of perinatologists in the central Virginia area. Pediatric inpatient consultations are provided at all Richmond area hospitals as well as at Riverside and Mary Washington hospitals.

For emergency cases, the VCUMC Life Evac helicopter is available for rapid and safe patient transportation.

The division also is dedicated to training future pediatric cardiologists and has an active ACGME accredited Fellowship Program for 20 years.

Community Pediatrics

The Division of Community Pediatrics includes every pediatrician in the community who has a relationship with the department. These physicians teach medical students in their offices, precept residents in their offices and clinics and serve on various ad hoc committees for the department such as search committees and the Bundy Professorship Committee.

Critical Care Medicine

Opened in 1978, the multi-specialty Pediatric Intensive Care Unit (PICU) is a state-of-the-art facility that accommodates seriously or critically ill infants, children and adolescents. There is a physician in the unit 24 hours a day, making it the only Level-1 PICU in Central Virginia. The PICU provides care for children with conditions, such as acute respiratory failure, status asthmaticus, circulatory shock, major trauma, head injury, intracranial hypertension, status epilepticus, diabetic ketoacidosis, renal failure and chemical intoxication.

The PICU also provides postoperative care for a broad range of surgical subspecialties such as neurosurgery, cardiac surgery, general surgery, transplant surgery, orthopedics, urology, plastic and ear-nose-and-throat surgery. Averaging 950 admissions a year, the PICU not only receives admissions from the emergency room, pediatric floors and operating rooms, but is a referral site for hospitals in the Richmond area and regional hospitals. The PICU has treated more than 18,000 children since 1978 and is proud of its team approach to care.

Thanks to a generous gift from Food Lion, the PICU recently unveiled its new, expanded and completely modernized facilities. The Food Lion PICU embraces the concept of "family-centered care" in the management of its patients. The new PICU has 12 critical care beds that have been configured to provide larger, more private patient spaces that can accommodate visitation by family 24 hours a day. The unit also provides a family resource area with Internet connections and a family shower area. The comfort of both patients and families figured prominently in the unit's design.

Developmental Pediatrics

Pasquale Accardo, M.D., a developmental pediatrician, offers evaluation and treatment consultation for children of all ages with developmental problems ranging from global developmental concerns to specific diagnoses. Assessments focus on the following diagnoses:

Endocrinology

The Division of Endocrinology provides comprehensive care for children with diabetes and all other endocrine disorders. The VCU Children's Medical Center is the major center in the region for the diagnosis and care of children with diabetes including insulin pump therapy. The Division of Endocrinology provides the only program in Central Virginia for children with endocrine disorders such as growth hormone deficiency, pituitary abnormalities including nodules and prolactinomas, hypothyroidism, hyperthyroidism, goiter, thyroid nodules or cancer, delayed or early puberty, congenital adrenal hyperplasia, polycystic ovary syndrome, metabolic bone disease, obesity, metabolic syndrome, endocrine hypertension, and hypoglycemia. Physicians in the division have great interest in children with Type II diabetes and obesity and have developed a multidisciplinary center for children with obesity and the metabolic syndrome.

Gastroenterology

Physicians at the Children’s Medical Center currently treat infants and children of all ages who have gastrointestinal problems, including reflux, ulcers, swallowing disorders and disorders of the liver and bowel such as hepatitis, chronic diarrhea, encopresis and inflammatory bowel disease. Sophisticated diagnostic procedures including endoscopy and pH testing are available at the Center.

General Pediatrics

Our dedicated team of general pediatricians offers preventative and primary care, such as immunizations and well baby/well child visits. Our physicians also provide diagnoses and treatment of acute illnesses in children of all ages while providing anticipatory guidance for their parents.

The Division of General Pediatrics works in partnership with our pediatric specialists to provide general pediatric care for children with chronic diseases or special health care needs. General Pediatricians play a large role in the Department of Pediatrics’ education and residency programs.

Hematology and Oncology

The mission of the Division of Pediatric Hematology and Oncology is to serve the Commonwealth of Virginia as a nationally recognized entity of clinical, educational, and research excellence. The clinic is dedicated to improving the quality of children’s life through prevention, control and cure of cancer.

The Division of Hematology/Oncology of the Children Medical Center has made, and continues to make, great strides in the battle against cancer. Cancers that were considered incurable 25 years ago now respond to treatments that produce tremendous survival rates. At the Children’s Medical Center, we are fighting the disease alongside children with cancer and their families. Our patients, ranging from infants to adolescents, receive state-of-the-art care from a dedicated multidisciplinary team of professionals in the ASK Pediatric Hematology-Oncology Clinic. 

Our Division of Hematology-Oncology is an active member of the Children’s Oncology Group and Pediatric Blood and Marrow Consortium. Members of our staff serve on multiple disease committees of the groups, and help design and implement new therapies for the fight against cancer.

  • The division’s COMPREHENSIVE SICKLE CELL CENTER is the medical home for more than 400 children with sickle cell disorders and other hemoglobinopathies. These children and their families face lifelong challenges and greatly benefit from the multidisciplinary team approach available here.
  • The HEMOPHILIA PROGRAM, serving children from the metropolitan Richmond area and beyond, provides a wide range of services to young people with hemophilia and other coagulation disorders.
  • The LONG TERM FOLLOW-UP PROGRAM was established in 2005 and has already registered approximately 400 patients. This multidisciplinary program focuses on screening and educating patients about late effect of cancer treatment, and improvement of quality of life.
  • THE NEURO-ONCOLOGY COMPREHENSIVE CLINIC, established in the spring of 2005, is a team comprised of an oncologist, neurosurgeon and radiation therapist all of whom work together to evaluate children with CNS tumors. This multidisciplinary approach enhances communication and provides comprehensive care from all services involved in the care of those patients
  • The PEDIATRIC STEM CELL TRANSPLANT PROGRAM in collaboration with the VCU Bone Marrow Transplant Center provides coordinated care for children requiring transplantation from life-threatening malignant and non-malignant disorders. The transplant program is a recognized COG allogeneic transplant center, a PBMTC member and an NMDP center.  .

Members of the division provide consultative services to physicians and other hospitals throughout Richmond and Central Virginia. Pediatric residents are involved in all of the division’s many clinical activities and gain experience in virtually all aspects of hematologic and oncologic diagnosis and care.

 A fellowship program was recently approved, and is planned to be operative in 2007. Division members are active participants in research through the Children’s Oncology Group and the Massey Cancer Center, a NIH-designated cancer center. 

In collaboration with other centers, they are also involved in clinical research in Sickle cell disease, Hemophilia and stem-cell transplantation. In collaboration with the VCU Department of psychology, members of the division also conduct studies in coping and quality of life of pediatric patients with cancer.

Infectious Disease

Services offered include inpatient and outpatient consultation as well as diagnosis and management of any problems related to infection including bacterial infections, viral infections, fungal infections, congenital infections, tick-borne infections, and tuberculosis. In addition, physicians are available for review of antibiotic and antiviral therapy, evaluation of Fever of Unknown Origin (FUO) and evaluation for immunologic conditions predisposing to infection, infections in catheters and other devices, gastrointestinal infections and lymphadenopathy.

A travel clinic is also available to address issues of infection and vaccination for families preparing for international travel.

Neonatal Medicine

Currently our Neonatal Intensive Care Unit (NICU) is the largest ICU at the VCU Health System and has the capacity to care for 40 premature infants and newborns. Under the direction of our neonatologists, fellows-in-training and all other pediatric specialists, the NICU cares for newborns and infants with a wide range of problems including all types of respiratory, surgical, cardiac, genetic, renal, neurological, hematologic and metabolic diseases.

Our physicians employ the latest treatment techniques to help smaller and smaller infants survive and thrive. In caring for infants requiring respiratory support, the NICU employs both high frequency oscillatory and jet ventilation, in addition to conventional ventilation. Nitric Oxide, Extracorporeal Membrane Oxygenation (ECMO) and surfactant replacement are also provided.

The physicians, nurses and other personnel in the unit are committed to “environmentally appropriate care” — controlling light, sound and stimulation. This commitment is reflected in the design of the new ICU (opening in 2007), which will consist of 40 stations with 36 rooms (four double suites). The new unit will include space for family “rooming in” with a sleeping area, storage and desk/work space.

Nephrology

The Division of Pediatric Nephrology is fully geared to provide services to children suffering from various acute and chronic renal disorders such as acute renal failure, chronic renal insufficiency, glomerulonephritidies, simple and complicated urinary tract infections, congenital renal abnormalities, voiding disorders, nocturnal enuresis, acid-base and electrolyte disorders, calcium, phosphorus and bone mineral disorders hypertension, and management of end stage renal diseases with hemo- and peritoneal dialysis. Facilities are available for renal transplantation and post-transplant care.

Palliative Care and Pain Management

Pediatric Palliative Care is multidisciplinary care focused on improving the quality of life and reducing distress and suffering of children with a complex or life-limiting condition and their families. This specialized care may be provided, in concert with curative treatments, from the time of diagnosis.

The multidisciplinary Pediatric Palliative Care and Pain Management (PPC) team works with the child’s primary team to provide pain and symptom management, assistance with complex medical decisions, counseling and psychological support.  The PPC team is available in both the hospital and the outpatient clinics. By embracing a child friendly, family centered approach, the PPC team promotes healing, comfort and hope.

Pulmonology

The VCUHS Division of Pediatric Pulmonology is the only Pediatric Pulmonary Group in Central Virginia. Our dedicated pediatric specialists provide in-depth diagnostic testing for patients who have chronic pulmonary problems such as asthma and cystic fibrosis. Following testing and diagnosis, patients of the pulmonary clinic are assured of expert care from our sub-specialists. Families are then given clinical and social services support to help them face the daily challenges of treating the young patient who has a chronic pulmonary disease.

The division provides multidisciplinary inpatient and outpatient services for infants and children with acute and chronic respiratory problems, including:

  • Asthma.
  • Cystic Fibrosis.
  • Bronchopulmonary disease (BPD).
  • Congenital lung abnormalities.
  • Neuromuscular respiratory diseases.
  • Chronic respiratory failure/insufficiency.
  • Pulmonary sleep disorders.

Many patients are now referred to the VCUHS Pediatric Pulmonary Function Laboratory for spirometry, lung volumes and exercise testing without a pulmonary consult.
