CD Services Resource Network (by geographical area)
The CD Services Resource Network members are persons in local communities available to help people with disabilities, their families, and others involved in the use of CD services in Virginia's Medicaid waivers.


CD Services Facilitators
The CD Services Facilitators are the providers contracted with the Department of Medical Assistance (DMAS) who are responsible for ensuring development and monitoring of the Consumer Service Plan, management training, and for reviewing activities as required by DMAS for CD Services.


DD waiver case managers
DD waiver case managers are individuals on behalf of the DD waiver case management organization who provide case management for individuals referred for the Individual & Family Developmental Disabilities Support Waiver. Case management includes ensuring the development, coordination, implementation, monitoring, and modification of the individual service plan, linking the individual with community resources, coordinating service providers and monitoring quality of care.


Medicaid Mentors (alphabetical)
Medicaid Mentors (by geographic area)
Medicaid waiver mentors are advocates in local communities available to help people with disabilities and their families through the Medicaid waiver process. The Mentors conduct workshops that explain Medicaid waivers. They may also address individual questions and concerns related to accessing waiver services.


Centers for Independent Living
Centers for Independent Living (CIL) are consumer-controlled, community-based, cross-disability, nonresidential, private, nonprofit agencies that are designed and operated in a local community by individuals with disabilities and provides an array of independent living services.


Community Services Boards
Community Services Boards and Behavioral Health Authorities are the local government agencies responsible for mental health, mental retardation, and substance abuse services for citizens in their communities.


RoadMap to Services
Virginia's RoadMap to Services was developed by the Department of Medical Assistance Services (DMAS) and is for people with disabilities or long term illness and the elderly. It is a directory designed to assist individuals with disabilities and their families gain full access to Virginia's health and human services.
