Graduation Forms and Instructions
These files are in PDF format. Adobe
Acrobat Reader is required.
MacOSX users: Preview will allow you to
view and print these forms, but will not allow you to fill out the form
electronically. Adobe Reader can be downloaded at
http://www.adobe.com/products/acrobat/readermain.html.
The following forms should be printed, completed and submitted together to
your advising office or department chair.:
One copy of your unofficial graduate transcript
One copy of the
Application to Graduate
- For Graduate Students (Parts I-II)
Instructions must be followed carefully. Complete your
Application to
Graduate - For Graduate Students only by typing in the PDF document, and then
print.
**School of Business candidates should submit their applications to Room B3102 in Snead Hall located at 301 West Main Street, or call 828-4622 to schedule an appointment.
**School of Allied Health Professions candidates should submit their applications to Monica White
in the Allied Health Professions dean's office.
**Candidates in all other programs should submit/review their applications
to graduate with their academic advisors.
The following forms should be printed, completed and submitted together to
your advising office or department chair. :
After
reading the
Undergraduate
Graduation Application Instructions , complete section A of your
Undergraduate Graduation
Application by typing on the PDF document,
and then print it out. For students with multiple majors, an application must
be completed for each major.
**College of Humanities and Sciences candidates should take their
applications with Section A completed to the secretaries in the offices of their majors.
**School of Business candidates should submit their applications with Section A completed to the Undergraduate Studies Office, Room B1100, in Snead Hall, or call 828-3710 to schedule an appointment.
**Allied Health candidates should submit their applications to Monica White
in the Allied Health dean's office.
** School of Nursing candidates should submit their applications with Section A completed to the Office of Enrollment and Student Services(Room 1009) in Theater Row.
**Candidates in all other Schools should review their applications with their
academic advisors after completing Section A.
If you
have already declared a minor, complete your
Graduation with Minor Application by typing
on the PDF document, then print it out. One copy must be submitted with your
Undergraduate Graduation Application. For students with multiple minors, an
application must be completed for each minor.
Students in Multiple Degree Programs
If you are applying to
graduate from more than one program, you must complete separate applications for
each program. Each application must be reviewed and approved by the appropriate
program director.
Selected VCU Medical Center Programs
DDS, Pharm D, and MD programs are not eligible for
checkout on e-Services, or using this site. Students in these programs should
contact their deans offices regarding applications to graduate.
